Community Outreach and Recovery Support Team - Compass Health

A specialty team that partners closely with many community partner agencies as part of a continuum of services, engaging clients seeking housing support who are either unenrolled from a health plan or seeking enrollment. The CORS Team aims to help clients access housing resources and other necessary services. Services in this program include Care Coordination, Housing Referral and Support, and Outreach and Engagement.

Rapid Rehousing - Northwest Youth Services

This program is for young people, ages 18-24, who are moving out of unstable situations that resulted in them being temporarily homeless. This program offers lease support, housing financial assistance, deposit, and ongoing skill building in collaboration with Housing Case Managers.\nNote: Rapid Rehousing is accessed through a main application process and is NOT first come, first served.

Peer Pathfinder Program - Lifeline Connections

Certified Peer Specialists provide recovery support to adults who have an opiate and/or stimulant use disorder and experience or are at risk of homelessness. Peer specialists have lived experience of substance use and mental health recovery and are trained to provide support services.

The Ground Floor - Northwest Youth Services

A day-use center for at-risk, runaway and homeless youth ages 24 and under. Youth can connect with supportive adults, receive basic needs items such as food, clothing and gear for sleeping outside, and connect with other resources that will help them end their experience of homelessness.

The PAD - Northwest Youth Services

Emergency housing program that offers housing for minors, ages 13-17 years for up to 90 days. Emergency housing helps youth reprieve and recover from crises, access critical services, and begin to set future goals for safety and stability.
