A specialty team that partners closely with many community partner agencies as part of a continuum of services, engaging clients seeking housing support who are either unenrolled from a health plan or seeking enrollment. The CORS Team aims to help clients access housing resources and other necessary services. Services in this program include Care Coordination, Housing Referral and Support, and Outreach and Engagement.

Max Higbee Center members are paired with a Community Engagement provider to receive one-on-one support to introduce community resources and create lasting relationships, develop goals and strategies, and learn skills that will facilitate integration into the community. An award winning, evidence‐informed home visitation case‐management model for pregnant and parenting moms with substance use disorders. This program serves pregnant and parenting moms who are self-disclosing to engaging in substance use during pregnancy. Pregnant and parenting moms may be admitted to the program from pregnancy or up to 24 months postpartum

Provides children’s mental health services to Medicaid eligible children and their families. After an initial assessment and determination of need for ongoing help, core mental health services available include individual and/or family counseling/therapy, medication evaluation/management and coordination with primary care physicians, and case management.

A voluntary program for current or former foster youth, aged 15 through 22, who would like to receive supportive case management to establish and achieve goals in various areas of their lives.

A home and community-based, high-intensity treatment service for adults whose recovery and success could benefit from more frequent contact with their clinical team. Services are designed to assist individuals with severe and persistent behavioral health disorders to accomplish their recovery goals and reside successfully in the community.

A community-based effort to find solutions for individuals who are high utilizers of criminal justice systems. The aim is to lead people out of the criminal justice system and into intensive case management where those who are dealing with behavioral health issues can have their needs met appropriately.
