Community Outreach and Recovery Support Team - Compass Health

A specialty team that partners closely with many community partner agencies as part of a continuum of services, engaging clients seeking housing support who are either unenrolled from a health plan or seeking enrollment. The CORS Team aims to help clients access housing resources and other necessary services. Maternity Support Services and Infant Case Management - Answers Counseling

Pregnant individuals and parents of infants who have Apple Health/ Medicaid health insurance are eligible to receive: assistance applying for SNAP/ Basic Food benefits, Childcare Subsidy, and PFML benefits; nutritionist advice; help accessing medical and behavioral health care; and monthly gift cards for participating in Culturally Responsive Integrated & Strength-Based Parenting (CRISP) classes.
